Use the following tutorial as a guide to help you apply for CDCP coverage. Make sure to read the instructions clearly before submitting your application. You must prepare your SIN number before starting your application as CRA will need it to verify your identity, your income as well as over all qualification for the Canadian Dental Care Plan. All the information you use on your application must be the exact same as the information you used to file your income taxes. If not, your application will be denied.

Upon completing your application, make sure that you save your application number. You will need that application number incase you want to follow up with your application with CDCP customer service.
